: Ethics approval was requested from the Ethics Committee of Northwest- and Central Switzerland (EKNZ) in Basel, Switzerland. In accordance with the EKNZ ethics approval was waived. In agreement with the same ethics committee, data was obtained using a non individualized hyperlink and the aim and purpose of the study were explained in the e-mail containing the link to the questionnaire. By clicking on the link and participating in the study participants gave informed consent. Non consenting recipients could simply ignore the link and e-mail.
