: This study was conducted in accordance with the World Medical Association’s Declaration of Helsinki. The present study was extracted from a PhD thesis registered with the project number 95–01–08-13440 and approved by the Research Ethics Committee of Shiraz University of Medical Sciences, Shiraz, Iran (Code: IR.SUMS.REC. 1396.S197). The study materials (interview questions and informed consent form) were approved by the ethics committee of the university. The participants were informed about the objectives of the study and the voluntary basis of their participation. We obtained written informed consent from all the participants.
