Declarations

: In Denmark, it is not possible to get a study evaluation from a scientific, ethical review committee in studies that are without patient contact and rely only on information from medical charts. Therefore, consent to participate from all the participants was not obtained since it is unnecessary. The Patient Safety Authority (reference number j.nr.31-1521-372) approved that information from the medical charts in the study could be retrieved without permission from the individual patient, and permission to handle and store the study data was obtained from the Danish Data Protection Agency (project nr.: P-2020-603). IRB approval in Denmark is deemed unnecessary according to national regulations.
: Obtained from Patient Safety Authority j.nr. 31-1521-372. Consent to publish from each patient is, for that reason, not needed.
