: The study was based on guidelines for ethical research as stated in the Declaration of Helsinki []. A willing, informed, written consent to participate was obtained from all participants. All methods were carried out in accordance with relevant guidelines and regulations: In Norway, the Regional Committees for Medical and Health Research Ethics (REC) are responsible for approving medical and health services research projects. When patient data is not involved in the project, the study do not need approval from REC [].
