Ethics approval and consent to participate
: We obtained ethical approval from the University of Ghana Institute of Statistical, Social and Economic Research (ISSER) Ethics Committee for Humanities (Ref. No. UG-ECH 057/13–14). We earlier sought official permission from the NHIA to use NHIS membership data for the analysis. In addition, informed consent was obtained from all participants who voluntarily participated in the study, after reminding them of their right to opt out of the study if they so wished.
