: Ethical committee authorization to the protocol was officially registered with a communication to the General Directorate n. DG1000/2484 the 15th of July 2005 ASL Lecce.(Local Health Authority of Lecce) as a part of the European Project CAREWELL - Multi-level integration for patients with complex needs (Grant Agreement No.: 620983).This study is part of the activities covered by the 2013–2015 Operational Program and subsequent updates of Department of Health of the Puglia Region with Deliberation of Regional Health Agency General Director (N° 122; December 5, 2015).In this study we use aggregated anonymous data were obtained from “Regional Health Authority Population Database”. Its use was regulated by an executive Act of Department of Health of the Puglia Region (N° 16; May 4, 2015). No informed consent is required using anonymous aggregated data (LD. June 30, 2003, n. 196 and Appendix ).
