Ethics approval and consent to participate
: Ethical approval to conduct the study was sought from the University of Limpopo Medunsa Research Committee which is a research ethics committee for University of Limpopo (MREC/HS/05/2013:PG). Institutional or departmental approval to conduct the study in health facilities was granted by the Provincial Research Committee which serves as the review board for the Department of Health in Limpopo Province. Written informed consent for participation was obtained from the participants and they were provided with information leaflet about the study and how should quality circles be conducted.
