Ethics approval and consent to participate
: An information letter about the aim of the study was sent together with the questionnaire to all participants. The letter also mentioned that study participation (i.e. completion of the survey questionnaire) was voluntary. The responses to the questionnaire were stored and analysed anonymously, in accordance with the Dutch Personal Data Protection Act. Formal ethical approval of this study was not required under the Dutch legislation, since research which requires filling in a questionnaire does not fall under the scope of the Medical Research Involving Human Subjects Act (WMO) when the research subjects questions are not burdensome and are not intimate (see ExternalRef removed).
