Ethics approval and consent to participate

: The Medical Ethics Committee of VU University medical centre assessed this study design and procedures, and in accordance with the local regulatory guidelines and standards for human subjects protection in the Netherlands (Medical Research Involving Human Subjects Act [WMO], 2005), this study proved to be exempt from further medical ethical review. HCPs consented to participate. The analysis of performance indicators was exempted from informed consent from the patients, since the electronic medical record data were anonymised and taken from the ANH database, which is carried out according to Dutch privacy legislation.
