Ethics approval and consent to participate
: The study received a favourable ethics opinion from Newcastle and North Tyneside1 Research Ethics Committee, Reference Numbers 12/NE/0059 and 12/NE/0284. All young people provided signed consent to join the study and for their medical records to be reviewed. For young people under sixteen years of age, a parent also provided signed consent for their child to join the study. All health professionals, parents and significant others gave signed consent to participate. All names in the report of the qualitative work are pseudonyms.
