Ethics approval and consent to participate
: The conduction of the evaluation was endorsed by the ethic review board of the Medical Faculty of the Technical University Aachen, Germany, as well as through the health authority of the city of Cologne and the administration of the German Red Cross Cologne. Since the researchers only assessed pseudonymized patient data from registers individual consent from visiting patients was not regarded as necessary. Patients during the participatory observation however gave oral consent. The observant was a medical student and signed a declaration to maintain all information confidential. All participants in the questionnaire based survey and the interviews gave informed consent.
