Ethics approval and consent to participate
: The study was approved by Regional Committee for Medical and Health Research Ethics, Norway, project No 2011/2574/REK sør-øst. Informed consent for participation in the study was obtained in verbal form from all participants, with additional consent from parents of patients from 13 to 15 years. No identifiable patient information was demanded by the study design, and signing and collecting consent forms would increase transparency, but also the risk for confidentiality breaks. The Ethical committee deemed that a procedure where we asked patients for oral consent before they produced anonymous questionnaire information and allowed using anonymized patient record information was acceptable in this study. The same considerations applied for consent from parents of patients under 16 years.
