: Ethical approval for the study was obtained from the joint University of Ibadan/University College Hospital Ethical Review Committee. Written informed consent was obtained from each of the participant at every stage of the study and confidentiality was maintained. There was no risk/harm to participants in this study. No identifier was attached to the data collection instruments. All the data were stored in a file that was pass-worded and only the relevant investigators were given permission to access the data for analysis.
