: This study was approved via the ethical committee approval number of 95-01-68-12101 by Shiraz University of Medical Sciences. There was no participants in the study process and only the formal published documents were analyzed.

: There was no difficulty in publishing the findings. The confidentiality of the documents was considered and findings are presented in a way that there was no concern for identifying people.
