Ethics approval and consent to participate
: This study was part of the large programme evaluation in Tanzania. Ethical approval for the evaluation study was obtained from the Institutional Review Board of the Ifakara Health Institute (approval number: 1BI1IRB/38) and the Ethics Review Board of the London School of Hygiene & Tropical Medicine. Study participants provided written consent to participate in this study, requiring them to sign a written consent form that was read out to them by the interviewers. This consent form was reviewed and approved by the ethics committees prior to the start of the research.
