Ethics approval and consent to participate

: Ethical approval was obtained from the NRES Ethics Committee London – Fulham (REC reference number 13/LO/0777) and an honorary contract was obtained from the East London NHS Foundation Trust for research to be carried out in the area of NHS Newham in London. The study was conducted in accordance with MRC Guidelines for Good Clinical Practice in Clinical Trials 1998 [CitationRef removed]. On the day of the interview, before each of the interviews took place, each participant was first asked to read and sign a consent form.
