Ethics approval and consent to participate

: The study was approved by the Faculty of Health Sciences, Faculty Human Ethics Committee, La Trobe University. The Chair evaluated the study as of negligible risk. Application reference number FHEC14/02.Informed consent was obtained from participants, either in writing prior to the interview being scheduled, or verbally if the interviewer requested to do the interview at the time of telephone follow-up. Verbal consent was recorded.
