: According to Swedish legislation [ CitationRef removed ], no formal approval from the Regional Ethics Review Board was needed for this project, but the ethics of scientific work [ CitationRef removed ] was followed in all steps of the work. The informants were included after verbal informed consent. They were informed - both verbally and in writing - that participation was voluntary and possible to withdraw at any time.
