: Authors GES, AJR, ShKS, StKS, and SJU were members of the Master of Health Care Delivery Science (MHCDS) Program at Dartmouth College Class of 2017. The HEART TRACKS Transitional Care Pathway was conceived as part of the MHCDS Program’s Action Learning Project and was implemented at GES’ institution. AJR, ShKS, StKS, and SJU served in an advisory capacity to SoutheastHEALTH during the development and implementation phases of HEART TRACKS. MH worked as the HEART TRACKS Coordinator during the study period, and EAH served as a Cardiology APRN provider who saw the majority of HEART TRACKS enrolled patients during the study period.
: Formal ethics approval was obtained from the SoutheastHEALTH Institutional Review Board. As the pathway was designed and implemented as a quality improvement initiative for all eligible patients, the institutional review board waived the requirements for informed consent.
