: Interview respondents provided written informed consent prior to interview. The London School of Hygiene and Tropical Medicine (ref:6305), Medical Research of Council Zimbabwe (ref: MRCZ/A/1676), and Biomedical Research & Training Institute (Zimbabwe) (ref: AP108/2012) gave ethical approval for the ZENITH trial, including qualitative data collection. The trial was also registered with the Pan African Clinical Trials Registry (PACTR201212000442288).
