Ethics approval and consent to participate
: The study was approved by the Research Ethics Committees of the South African Medical Research Council (EC002–2/2014) and Stellenbosch University (N14/02/008). The informed-consent form was sent to the facilities prior to the FGDs and was also explained and confirmed at the start of the FGDs. All participants provided individual written informed consent. Participants were advised that information shared in FGDs has the potential to be more widely shared, and therefore we encouraged participants to respect privacy and confidentiality. The names of FGDs have been captured and has restricted access, and any identifying information were redacted from all transcripts.

: TK has contributed evidence to the National Department of Health Essential Drugs List Adult level standard treatment guideline (non-funded); and facilitated workshops and capacity development for under and post-graduate students, researchers, policymakers and practitioners on clinical practice guidelines and evidence-informed practices. JV has been involved in guideline development globally and regionally, he has been on advisory committees for clinical guidelines in the Western province, and has facilitated workshops and capacity development for under and postgraduate students, researchers and practitioners on clinical practice guidelines and evidence-informed practices.
