Ethics approval and consent to participate
: The study was approved by the University of Witwatersrand Human Research Ethics Committee (Protocol Reference No: M140534) and The Alfred Hospital Ethics Committee, Australia (Project No: 256/14). Written informed consent for study participation and for audio recording was obtained. As the sample size was small it is potentially possible to identify the respondents. Participants were informed of this risk and permission was specifically requested for using quotes from the interviews if they were attributed to a cadre of health worker (e.g. “nurse”, “laboratory worker”).
