: DH was the incumbent of the position described in this study, at the time of the research. LV and KL were senior occupational therapists at the service, and GP and KS were senior staff at the partner university.

: This study received ethical approval from the Melbourne Health Human Research Ethics Committee (QA QA2014029). All participants received a plain language statement, and they were informed consent was assumed if they returned a response to the survey.
