Ethics approval for the ZAMSTAR and SOCS studies were obtained from the Biomedical Ethics Committee at the University of Zambia, Lusaka, Zambia; the Stellenbosch Health Research Ethics Committee, Tygerberg, South Africa; and the London School of Hygiene & Tropical Medicine, London, UK.
