Ethics approval and consent to participate

: Ethical clearance was obtained from the Higher Degrees Committee of the Department of Health Studies at the University of South Africa (UNISA). The Addis Ababa Regional Health Bureau Ethical committee, in Ethiopia, provided ethical clearance to provide the study in the country. Letter to conduct the study in the health facilities were obtained from sub-city administration health departments and the respective health facilities. Written consent was obtained from all study participants.
