Ethics approval and consent to participate

: The Children’s National Health System Institutional Review Board (IRB) acknowledged the study since it met the criteria of a quality improvement effort per our policies and did not require standard IRB review. The need for review and consent were waived by the Children’s National Health System IRB. This research is based on survey results by participants from around the country and would not be feasible with a regular consent process, hence the Institutional Review Board approved a waiver of consent. All surveys were de-identified.
