Ethics approval and consent to participate
: The evaluation protocol was reviewed by the University of Bristol Research Ethics Committee, classifying this health service evaluation as an audit of existing NHS procedures not requiring further ethical approval. All interviews were conducted in accordance with data protection and confidentiality regulations, by briefing participants and obtaining written informed consent.
: Written consent was obtained from participants to include anonymised direct quotations from interviews in subsequent publications.
