Ethics approval and consent to participate
: This study was approved by the Human Subjects Research Committee of the University of Washington (STUDY00001851), the Kenyatta National Hospital-University of Nairobi Ethics Research Committee, and Mombasa County. Clinic managers that were interviewed provided verbal assent to participate in the interview. There was a waiver of written consent for family planning clinic clients. These were both approved by all IRBs.
