Ethics approval and consent to participate
: The research protocol described in this paper as well as all the relevant data collection tools were reviewed by two independent ethical review boards, obtaining ethical clearance both from the Ethics Committee of the Medical Faculty of the University of Heidelberg (Protocol number S-272/2013) and Burkina Faso National Ethics Committee (Protocol number 2013-7-06). All survey respondents both at the health facility and at the community level were carefully informed about the study and signed informed consent forms prior to data collection. Since the study did not entail administration of any medical treatment, study participants were not exposed to any relevant risks for their health. The possible disadvantage of residing in a control vs. an intervention district was minimized by the expectation that the intervention could be scaled up following completion of the pilot. As PBF was introduced as an intervention to strengthen the existing health system, a lack of exposure to PBF did not expose health workers or patients to additional risks.

Nevertheless, they declare having interacted in full autonomy with the independent team of researchers at Heidelberg University and Centre Muraz and not having received any pressure from higher level management in relation to the definition of the study design.
