Ethics approval and consent to participate
: The study was approved by both the Human Research Ethics Committee (HREC) at Melbourne Health and the HREC at the University of Melbourne. Participation was confidential, and managers were not informed about who participated. Informed consent was obtained via a Plain Language Statement. Participants were asked to sign and return a consent form prior to commencing an interview.
