Ethics approval and consent to participate
: The project was categorized as quality improvement and/or program development and was subsequently waivered from further IRB at the University of California-Davis. The questionnaires had introductory remarks about the survey, and a statement that clearly stated that participation in the survey was entirely voluntary, and that the information obtained was to be kept anonymous and confidential. Therefore, consent to participate in the survey was implied by responding to the survey questions. Using the Survey Monkey system, it was not possible to know who the respondent was, and hence confidentiality was in-built. As this study involved no patients or personal information, research ethics approval was not required.
