Ethics approval and consent to participate
: Approval was obtained from University of Cape Town Human Research Ethics Committee (568/2017), City of Cape Town Research Ethics Committee (7876) and the Western Cape Provincial Health Research Committee (WC_201709_011). All research participants provided written informed consent before any data collection commenced. Where participants were children (under 16 years old), written informed consent was obtained from caregivers (parents or guardians). Children over 7 years old were also asked to give assent to their participation.
