: This study was approved by Partners Healthcare Institutional Review Board (2012P001069) and the Navajo Nation Human Research Review Board (NNR-11.150 T). Consent to participate was waived for this observational study by both ethics committees. In order to abstract data, study staff obtained necessary certification and training required by the Indian Health Services Rules of Behavior under the auspices of Collaborative Agreements with participating sites (NV-CA-10-0011, NA-CA-13-0013, NA-CA-11-0062).
