Ethics approval and consent to participate
: This study was completed as part of a larger government Program Evaluation and Quality Improvement project that was reviewed by the Western University Research Ethics Board (REB). The REB considered the project not to be research as described in the Canadian Tri-Council Policy Statement V.2 (Research Exempt from REB Review, Article 2.4) and therefore it was not considered to fall under the purview of the REB. Although consent was not required by the REB, due to the nature of this study, verbal consent to participate and to be audio-recorded (where applicable) was obtained from all participants.
