Ethics approval and consent to participate
: All eligible participants were offered a survey by the registration clerk upon presentation. All gave informed written consent to complete the questionnaire and to have their electronic medical record accessed for further information. A subset of participants consented to be contacted later and the twenty-five qualitative interviews presented here were conducted as part of that follow-up and it is this data that is presented here. Participants were offered in person or telephone interviews. Telephone interviews were predominantly used due to challenges arranging appointment times and participant transportation. Prior to interview, participants gave informed written (in-person) or verbal (recorded over the phone) consent. The data from the larger quantitative component of the study is pending publication and available upon request. All components of the study including the verbal consent script were approved by Queen’s University Health Sciences and Affiliated Hospitals Research Ethics Board.
