: The English diabetes survey was approved by the NHS National Research Ethics Service (Skipton House, 80 London Road, London SE1 6LH). The Finnish survey was approved by the Ethics Committee of the Central Finland Health Care District. The German survey was approved by the Ethics Committee of the Medical Faculty of the Friedrich-Alexander University in Erlangen-Nürnberg. The Greek survey was approved by the Scientific Committee of the hospital in Heraklion and the Greek stroke survey by the Ethics Committee of the hospital Alexandra. The Dutch survey was approved by the board of directors of the Primary Care Group ZEL. The Spanish survey was approved by the Hospital La Fe Ethical Committee.Participants gave their consent to participate by voluntarily handing over or voluntarily sending by mail the completed questionnaire. Survey participants were completely anonymous to the study researchers.
