Ethics approval and consent to participate

: The study was submitted to the Regional Committees for Medical and Health Research Ethics (REC) in Norway, study number 2017/2073/REK midt (Trondheim, Norway), who determined that the study did not require approval because it does not comprise medical research. The Norwegian Centre for Research Data (NSD) was notified, number 57803. Transcripts and audio recordings were kept in Boxcryptor, and only the first author had admission to it. The ethical principles emphasised in the Declaration of Helsinki were followed []. Participation was voluntary, and a written informed consent was required from each informant. The informants could withdraw at any time. The informants were adult and competent to give consent, and the entity for the study should not be perceived as being sensitive.
