: Institutional Review Boards (IRB) of the five participating Veteran Affairs healthcare systems gave permission for the study on November 9, 2011. The Clinical Trial Registration, NCT01532986, was listed on January 13, 2012. The IRBs waived the requirement for documentation of written consent and allowed verbal consent for the participants (veterans).
