Ethics approval and consent to participate
: As the study was part of a process evaluation, ethical approval was not required according to the Swedish Ethical Review Act (SFS 2003:460). The study was conducted according to the ethical principles of the Helsinki declaration [CitationRef removed]. All participants were given verbal and written information about the study and about participation being voluntary, and signed a written consent. Findings are presented so that no individual participant can be identified.
