Ethics approval and consent to participate
: The study was approved by the Regional Committee of Medical and Health Research Ethics (ref: 2014/435) and the National Committee for Medical and Health Research Ethics (ref: 2014/160), both of which are appointed by the Norwegian Ministry of Education and Research. It was also approved by the Privacy Ombudsman at Akershus University Hospital (ref: 13/138). Written consent was given by all participants prior to interview.
