Ethics approval and consent to participate
: This study was conducted according to the principles of the 1996 Declaration of Helsinki [CitationRef removed]. The study was approved by the Regional Ethical Review Board committee in Uppsala (ref: Dnr 2017–195, 2017–195 1B). All participants were given written information about the study and gave their written informed consent to participate. Participants who were involved in interviews were given information about the interviewers and note takers role in the project and their reasons for doing the research.
