Ethics approval and consent to participate
: An ethical clearance was obtained from the Human Ethics Committee of the University of Canterbury (HEC 2015/139). In addition, an approval was also obtained from the Nepal Health Research Council (NHRC). Written consent was obtained from the medical superintendent or official in charge of each of the thirteen selected hospitals before initiating study activities in the hospitals.
