: University of Minnesota Institutional Review Board (1512 M80888).Emory University Institutional Review Board (IRB00094148).The University IRB determined that discussions with clinic staff were quality improvement activities and not research. As such, written informed consent procedures were not required. However, verbal consent to audio record was sought by the interviewer prior to each discussion with clinic staff. All clinic staff agreed to the audio recordings.
