: All client participants gave written consent to be involved in the study and received a $50 voucher for participation. Ethics approval for the study was granted by the Research Ethics and Governance Office, The Royal Prince Alfred Hospital, Sydney Local Health District (X15–0138 & HREC/15/RPAH/190).

: No competing financial interests. The primary researchers were employed by Sydney Local Health District and affiliated with the Healthy Homes and Neighbourhoods Integrated Care Initiative. Researchers JE and EM are line-managers of the clinical staff providing the Healthy Homes and Neighbourhoods initiative.
