Ethics approval and consent to participate
: We received ethical approval to conduct formative research, including engagement with stakeholders during a one-day meeting, from institutional review boards at the Kenya Medical Research Institute (KEMRI) and University of Washington (UW). Because stakeholders were involved in the collaborative development of the care pathway and were not research subjects themselves, the need for individual stakeholder consent was waived by the KEMRI and UW institutional review boards. All stakeholders reviewed and approved this manuscript for publication and have been acknowledged for their participation in the care pathway development process.
