Ethics approval and consent to participate
: Favourable ethical opinion for the research has been obtained from the University College Dublin Research Ethics Committee (ref: HREC-LS-16-116397) and the participating hospital sites. All participants provided written informed consent during each phase of data collection and all potentially identifiable characteristics were removed from each transcript and pseudonyms have been assigned to both cases to maintain anonymity.
