Ethics approval and consent to participate
: Because patients and healthcare professionals were involved in this study, efforts were made to carry it out according to current laws, regulations, and codes of research ethics. Therefore, and prior to the study, permission was obtained from the Regional Ethical Review Board in Gothenburg, Sweden (Dnr; 1006–16). Managers of the patient process organisation at the hospital involved also approved the study. Process leaders approved the first author (CB) participating in the QI team meetings. Informed, written consent was obtained from the interview informants.
