: The study was approved by the Regional Committee for Medical and Health Research Ethics, Norway (project 2011/790 REK sør-øst), and by the Data Protection Officer at Akershus University Hospital (project 2012_097). Patients received oral and written information about the project, and only patients giving written consent were included. Refusal to participate had no consequence in the care offers.
