Ethics approval and consent to participate
: The study followed the Helsinki declaration’s research principles []. The Regional Committee for Medical and Health Research Ethics (REC West) of the Western Norway Health Region (2016/1102) approved the study prior to study start. Patients were given verbal and written information about the study at recruitment. The participants were informed that their participation in the study was voluntary and that they could withdraw at any time without consequences. All participants signed an informed consent form.
