Ethics approval and consent to participate
: This research was approved by Human Research Ethics Committee at The University of New South Wales (UNSW) Sydney (HC17586) and Research Ethics Committee (REC) at Addis Ababa Regional Health Bureau, Addis Ababa (A/A/H/13/2122/227). Information, including the purpose of the study, the content of the interview, participant rights not to take part in the study or to stop the interview at any time were explained to each participant. Participation was voluntary, and all informants provided a verbal informed consent to take part in the study as well as to being recorded during the interview. A verbal informed consent procedure deemed appropriate for the current study as there were participants who were unable to read and write. This procedure was approved by the two ethics committees. Privacy and confidentiality were addressed with the participants interviewed in a private room and all files assigned anonymous participant ID.
